Stowe Cardiology is pleased to welcome new patients. Dr. Daum accepts Medicare and most health insurance plans. We'll be very pleased to help you with insurance questions and issues related to referrals -- please just ask. Any co-pay required by your health plan is due at the time of your visit.

Before your first appointment, you'll need to complete a registration form. You may do this when you arrive for your first visit, or you may print the new patient registration form and fill it out ahead of time. You also may fax it to us in advance. Since the form contains confidential personal and medical data we do not provide for online processing of the form.
